Qualifications

Required Education and Experience

• Bachelor’s degree or an equivalent combination of training and experience

• Two years of related experience

Preferred Qualifications

• Six years of related experience in public health, health outreach and education

• Experience in OB related care management

Knowledge, Skills, and Abilities

• Ability to speak, read, write English and Spanish

• Knowledge of word processing, spreadsheet, and database applications

• Knowledge of health care education

• Ability to multi-task and work cooperatively with others

• Ability to work with sensitive information and maintain confidentiality

• Strong interpersonal and organizational skills

• Strong verbal and written communication skills

Responsibilities

• Day-to-Day Functions: Lead and coordinate day-to-day activities with direction from the Program Director of the HRSA maternal services grant. Performs higher level duties and supervision of Community Health Worker. Day-to-day functions will work to implement changes for grant success with guidance from Program Director. Assists in establishing short- and long-term goals, objectives, policies, and procedures. Assists with strategic planning. May supervise program staff. May assist with training and materials.

• Patient Outreach and Recruitment: Develop protocols and guidelines for determining appropriateness of telehealth in maternity care to better serve remote rural areas currently not offering maternity care services. Collaborate with community resource centers in rural areas to create a directory of all resources available to pregnant and postpartum women, including those with IPV; and conduct chart reviews to ensure patients receive the available resources, and participate in interdisciplinary team huddle for the most complex patients to ensure all needs are met. Develops and maintains relationships with stakeholders.

• Patient Coordination & Clinical Support: Communicate with and educate patients on their risk level and why their condition may preclude them from virtual visits. Assist with expansion of prenatal care into the surrounding rural communities. Includes coordinating and assisting in patient scheduling for rural patients in Navasota and other rural communities.

• Documentation and Data Collection: Assist in implementing the identified social determinants of health and mental health screening priorities into a user-friendly template in the TAMH-FCC electronic medical record and train providers and staff on using the template. Prepares reports, maintains records and databases. May assist with budgetary items.
