📋 Description

• Provide medication management, reconciliation, and adherence counseling per protocol.
• Collaborate with the care team to develop individualized therapy plans.
• Monitor regimens to meet therapeutic goals and manage adverse effects.

• Support quality initiatives, REMS compliance, and process improvements.
• Optimize workflows and assist with prior authorizations and approvals.

🎯 Requirements

• PharmD and CA pharmacist license required.
• Advanced Practice Pharmacist in CA via two of: ACPE cert, residency, or 1500 hours.
• 3+ years of clinical pharmacist experience; PGY1/PGY2 residency preferred.
• Strong pharmacology knowledge, FDA regulations, and evidence-based care.
• Excellent communication, organization, and teamwork.
• Ability to educate patients and support medication safety.
