📋 Description

• Visit clients in their homes and supervise field clinicians.
• Manage internal casework and develop field staff.
• Assist with in-home client assessments and care planning.
• Provide clinical insight to support the care planning process.
• Work with patients and office staff.

🎯 Requirements

• Current New Hampshire RN license in good standing.
• Minimum two years field experience in a Medicare-certified Home Health agency; includes OASIS, ICD-9, QA, and case management.
• Strong assessment and organizational skills.
• Demonstrated ability to work independently with diverse responsibilities.
• Ambition to grow beyond current position.
• Solid communication and PC skills.
