📋 Description

• Provide comprehensive medication management, reconciliation, dose optimization, adherence support, and drug-level assessments aligned with evidence-based standards.
• Partner with physicians, specialists, and interdisciplinary teams to deliver individualized care plans and seamless medication coordination.
• Monitor therapy safety and effectiveness; identify optimization and cost-saving opportunities; support quality-improvement and REMS initiatives.
• Streamline operations by supporting complex prior authorizations, improving workflows, educating patients, and maintaining ongoing clinical development.

🎯 Requirements

• PharmD degree
• Licensed pharmacist in CA
• Plus any two of the following: Board certification in a relevant field; Postgraduate residency with direct patient care; At least 1 year of clinical services under a collaborative practice
• 3+ years clinical pharmacist experience or PGY2
• Strong communication, organization, and tech skills
• Empathy, integrity, and a team-first mindset
