📋 Description

• Conduct initial assessments to determine care needs and plan care.
• Train and supervise non-skilled caregivers in personal care.
• Monitor clients' wellbeing and report changes to healthcare professionals.
• Educate clients and families on effective care strategies and safety.
• Ensure compliance with agency policies and state regulations.
• Maintain accurate documentation of client care and progress.

🎯 Requirements

• BSN or nursing degree with NYS license.
• 1-2 years of nursing experience in homecare.
• Experience in geriatric care or with disabilities.
• Ability to travel within the five boroughs as needed.
• Knowledge of state and federal home health regulations.
• Strong interpersonal and communication skills; MS Office proficiency.
• Strongly preferred: Spanish, Russian, Chinese, Korean, or Bengali.
• Proficient in electronic health record (EHR) systems.
• Valid driver's license and reliable transportation.
