📋 Description

• Provide comprehensive medication management and reconciliation.
• Partner with physicians and teams to order care plans under CPA.

🎯 Requirements

• Hold a Doctor of Pharmacy (PharmD).
• Must be licensed pharmacist in Washington.
• Completion of PGY2 Oncology Residency.
• PGY1 Ambulatory Care Residency + at least 3 years oncology experience.
• Have at least 3 years of oncology pharmacist experience, CPA experience.
• Strong communication, organization, and tech skills.
• Empathy, integrity, and a team-first mindset.
