📋 Description

• Provide frontline care to patients experiencing homelessness and related conditions.
• Manage patient panels with acute, chronic, and preventive care in the field.
• Assess patients, histories, exams, order tests, and document findings.
• Report to supervising physician and exercise independent judgment within state regulations.
• Design and implement care plans to support stability and improved outcomes.
• Collaborate with a multidisciplinary team to coordinate care.

🎯 Requirements

• Active, unrestricted California NP certificate by hire.
• California NP Furnishing Number (CA).
• California NP National Certification.
• California RN License.
• Travel up to 75% with valid CA Driver's License and insured vehicle.
• Comfortable delivering direct care in outdoor/field settings.
• Strong interpersonal and communication skills for diverse populations.
• Independent clinical judgement within scope and regulations.
• Preferred: Bilingual English/Spanish strongly preferred.
• Preferred: 1+ year experience with CalAIM populations.
